We provide assistance with the assessment of client abilities and preferences, the planning of a loved one’s future care needs, the development of a care plan, the provision of information and referrals, and the provision of support during the admission process. We help clients with budgeting and income management and assist with referrals and information on financial services, employment benefits, and government programs. We also offer workshops on topics such as how to invest, how to buy a house, and how to make smart decisions with your money. Some of our clients are seniors, people with disabilities, low-income earners, or people on social assistance, and we help them navigate the maze of available programs and navigate through the paperwork involved to make sure they are getting the most out of the government programs that are available to them.

Mobility Equipment

We provide the necessary equipment for clients to safely and independently navigate their homes. We assist with the appropriate licensing for the equipment.

As a licensed non-emergency medical equipment supplier, we are able to provide the proper documentation to our clients. This ensures that all equipment is installed according to code and standards. We can assist with the installation of any necessary equipment. For example, an elderly person may need a lift installed to enter their home safely. Care Services of Hope disability support is designed to be a supportive and integrated program that provides medical and social services together. We work together to provide a comprehensive and coordinated approach to health care. We provide medical services, including primary care, medication management, and health assessment. We also provide social services, including assessments, information, care planning, and referral to other community resources. We also provide a range of services for people with disabilities and aging. Care Services of Hope disability support is provided by a team of professionals, including medical doctors, nurses, and occupational therapists. Through collaboration with other community partners, individuals with disabilities may also receive health assessments and medication management through the Community Care Access for Persons with Disabilities program.

In some cases, the local government may cover some of the expenses associated with your Care Services of Hope disability support.
Care Services of Hope disability support services take place in the communities in which individuals live. This may be where you live or in a community where you have access to essential support. We provide assessment and management services to community partners and work with these partners to support individuals with disabilities. We also provide home support to people in residential care facilities, assisted living communities, and other settings that are appropriate for home support.
