What is Care Services at Hope disability support?
Care Services is a comprehensive program that offers a full range of support for people with disabilities. Our goal is to support individuals through the transition from health services to long-term care. Our services fill a gap between the health and social services that individuals receive and the long-term care services and support that are most appropriate for a person’s needs. We are made up of medical services, nurses, and occupational therapists. Our medical services include primary care, medication management, and health assessment; our nurses specialize in geriatric assessment and management of health conditions; our occupational therapists specialize in assisting with the management of health conditions and assist with mobility needs and functional assessment.
How does Care Services of Hope disability support work?
Care Services of Hope disability support is designed to be a supportive and integrated program that provides medical and social services together. We work together to provide a comprehensive and coordinated approach to health care. We provide medical services, including primary care, medication management, and health assessment. We also provide social services, including assessments, information, care planning, and referral to other community resources. We also provide a range of services for people with disabilities and aging. We include assessment and management of health conditions; assistive technology services, including assistive devices and assistive software; assistance with navigating community supports, including home care and long-term supportive services; assistance with financial management, including assistance with accessing government programs; and education and training on health conditions and disability management.
Who provides Care Services of Hope disability support?
Care Services of Hope disability support is provided by a team of professionals, including medical doctors, nurses, and occupational therapists. The medical team is led by a medical doctor, who works closely with a medical nurse to provide medical care. The medical nurse provides care under the direction of the medical doctor and is responsible for coordinating the health assessment and medication management related to each individual’s health condition. The medical team may include a medical assistant, pharmacist, or primary care physician who is available to coordinate care. The medical team may also include a gerontology nurse who specializes in older adults or a geriatric nurse who specializes in care for people with disabilities. Through collaboration with other community partners, individuals with disabilities may also receive health assessments and medication management through the Community Care Access for Persons with Disabilities program.
