Care Service

Our Disability Care Service is for people who receive care. In this role, you will help your disabled loved one access medical appointments, provide transportation for medical appointments, assist with access and independence needs, assist with records and information management, home maintenance and renovations, assist with financial management, and other services related to access and independence.

Transport Service

A Disability Transport Service provides mobility and transportation needs of a person with a disability. This may include wheelchair assistance, scooter assistance, or any other mobility issues. The role may include transporting the person to medical appointments, grocery stores, church functions, or other community events. The transport service will also help the person with accessibility needs such as ramps, door thresholds, and other elevations to help the individual maneuver and access their requested locations.

Housing Service

Our Disability Housing Service is for people who receive care. This may be a short-term stay in a nursing home, a long-term stay in a residential care facility, or any other type of in-home care. The Housing Service can help you navigate the medical and care system and ensure your loved one gets the assistance they require to stay in their home. The Housing Service will gather documentation from doctors, provide transportation for medical appointments, schedule home maintenance appointments, assist with finances, help with information management, and more.

Care at home service

Our Hope Disability Care at home service is for people who receive care. This is a remote service that provides support to live at home while managing a disability. The person receiving care will receive assistance with daily tasks including bathing, dressing, eating, and personal care. The support person will help with medication management, grocery shopping, paying bills, paperwork, and other assistance as needed. The service is flexible to the needs of the client receiving care.

Community service

Our Hope Disability Community Service is for people who receive care. This is a remote service that helps with daily living activities. The person receiving care will receive assistance with managing their disabilities such as managing medications, communication, mobility, and ability to get out of the home environment. The service will assist with meal preparation, transportation, scheduling medical appointments, arranging medical equipment, and more. The service is flexible to the needs of the client receiving care.

Our Hope Disability Health Service is for people who receive care. This helps with the medical needs of a person with a disability. The Health Service will help with accessing medical appointments, and medications, managing medical conditions, and will assist with navigating the medical system. We may also provide services related to communication. For example, we may provide sign language interpretation, assist with computer needs and assist with communication needs.
